How diabetes and prediabetes are diagnosed

These are the diagnostic ranges used by the American Diabetes Association. Unless you have clear symptoms of high blood sugar, a diagnosis is normally confirmed by a second abnormal result — either the same test repeated or two different tests.

A1C can be misleading with anemia, pregnancy, recent blood loss or certain hemoglobin variants. Your TopInspired specialist will choose the right test for you. The NIDDK explains how each diabetes test works.

What does an endocrinologist do?

An endocrinologist is a physician who specializes in the endocrine system — the glands and hormones that regulate metabolism, energy, growth, reproduction and bone health. At TopInspired, our endocrinology team diagnoses and manages conditions such as type 1 and type 2 diabetes, thyroid disease, PCOS, osteoporosis and adrenal or pituitary disorders, usually working alongside your primary care doctor.

What is TopInspired.com?

TopInspired.com is the website of TopInspired, a specialist-led endocrinology and diabetes practice. It is where you can read our condition guides, see how a consultation works and request an appointment with an endocrinologist, diabetes care and education specialist or registered dietitian — in person or by secure video.

Do I need a referral to book an appointment at TopInspired.com?

Many patients can book directly. Some insurance plans — particularly HMOs — require a referral from your primary care provider before a specialist visit is covered, so check your plan first. If you do have a referral letter, send it with your recent lab results so we can prepare for your first consultation.

What should I bring to my first endocrinology appointment?

Bring photo ID and your insurance card, a list of every medication and supplement you take (with doses), recent lab results such as A1C or thyroid tests, and your glucose meter, CGM app login or logbook if you have diabetes. Writing down your questions beforehand helps you get the most from the visit.

Can I see a TopInspired.com specialist online?

Yes. Many follow-up visits — reviewing CGM data, adjusting medication or discussing lab results — work well over secure video. A first visit, or any visit that needs a physical exam such as a thyroid or foot check, may need to be in person. Virtual care availability can also depend on the state you are in.

Can type 2 diabetes go into remission?

For some people, yes. An international consensus report led by the American Diabetes Association defines remission as an A1C below 6.5% that is maintained for at least three months after stopping glucose-lowering medication. Substantial weight loss is the strongest predictor. Remission is not a cure, so regular monitoring continues — your TopInspired specialist can tell you whether it is a realistic goal for you.

How often should my A1C be checked?

The ADA Standards of Care recommend checking A1C at least twice a year if you are meeting your treatment goals, and about every three months if your treatment has changed or you are not yet at goal. If you use a CGM, your specialist will also review metrics such as time in range.
